All patients must have a valid, current recommendation from their doctor prior to registering as a patient with Harborside Health Center. If you have a current recommendation, you can register online or in person at either of our locations. All patient recommendations will be verified prior to any purchase or visit to either of our locations. Recommendations must be current when placing orders online or visiting us in person.

If you are a NEW patient and making your first purchase through our delivery service, you must fill out and have ready the New Patient Agreement Form upon your first order.
